Social Worker

We have an exciting opportunity for an experienced Social Worker to join our Family Support Team in a permanent 0.5 FTE role (20 hours per week).

About the role

This role offers a variety of working environments—meeting patients and whānau in our Inpatient Unit, visiting them in their homes, or supporting them in an Aged Residential Care facility. You’ll play a key role in ensuring patients and their families/whānau receive psychosocial support, care planning, and practical guidance as they navigate a pivotal time in their lives.

Your mahi will include psychosocial assessments, care plan development, and case and risk management. You will also provide consultation, liaison, education, and support to other services caring for people with palliative needs.

You’ll be part of an interdisciplinary team of dedicated professionals whose shared focus is the care, comfort, and dignity of those we support.

We welcome applications from people who bring both professional expertise and personal warmth to their work—those who value connection, cultural safety, and teamwork.

About you

To be successful in this role, you will have:

  • A qualification in Social Work recognised by the NZ Social Workers Registration Board.
  • Full registration with the NZ Social Workers Registration Board.
  • Strong clinical knowledge and experience, ideally but not exclusively within a healthcare or hospice setting.
  • A solid understanding of relevant legislation, standards, and ethical frameworks.
  • The ability to work collaboratively within a multidisciplinary team.
  • Excellent communication skills—written, verbal, and interpersonal.
  • A flexible and motivated approach to your work.
  • Demonstrated commitment to cultural safety and partnership under Te Tiriti o Waitangi.
  • Empathy, professionalism, and a commitment to excellence in palliative care.
  • Digital literacy and confidence using electronic records.
