To access our services, you’ll need to be referred and accepted into our care. First-time referrals and re-referrals can be made by the person, whānau, a loved one, or a health professional.
To be referred to Cranford Hospice for care, the person needs to:
- agree to the referral, or their Enduring Power of Attorney (EPOA) agrees on their behalf.
- have an advanced terminal illness.
To begin a referral, choose the button that applies to you, then complete and submit the correct form.
Who are you referring?
If you’re a health professional and would like to refer a patient, please complete the referral form in PalCare.
Referrals are processed Monday to Friday, and one of our team members will be in touch within one to two working days.
Self-referral
If you're referring yourself, please complete and submit this form.One of our team members will be in touch within one to two working days to discuss your referral.
Referring someone else
If you're referring whānau or a friend, please complete and submit this form. One of our team members will be in touch within one to two working days to discuss your referral.
