Advance care planning is a process of thinking and talking about your values, goals and preferences for current and future health care. It helps you understand what the future might hold and say what health care you would or would not want, including end-of-life care.

Advance Care Planning

Tō tātou reo advance care planning (ACP) helps your whānau and other important people in your life, along with your health care team, understand your values and goals and what your preferences are for current and future health care. It ensures that what matters most to you is known and respected.

An ACP can cover medical care, as well as other important aspects of your life.

  • What makes life precious and meaningful to you, where you’d like to be and who you would like to be with throughout your palliative care journey. 
  • How you want to be cared for spiritually and emotionally. 
  • Whether you want a funeral and what that funeral would look like, whether you want to be buried or cremated. 
  • Your decision around organ donation. 
  • Where your important documents are kept and whether you have an Enduring Power of Attorney or Advance Directive in place. 

We can guide you through the process of creating an ACP and help you to select a template that best meets your needs.
