Support for patients
We provide specialist palliative care to people living with a life-limiting illness such as cancer, heart failure, or neurological, kidney and lung diseases that cannot be cured.
Palliative care focuses on helping you live as comfortably and fully as possible, while supporting you and your whānau as your needs change, including at the end of life.
Specialist palliative care is needed when symptoms are complex or difficult to manage. Our team has additional expertise and may use medications and approaches not commonly provided by other health services, always with a focus on quality of life.
Referral required
To receive care from Cranford Hospice, you need to be referred and accepted into our service. You, a family member, friend or health professional can make a referral. Make a referral

Where you will receive care
Home
Most people are cared for in their own home or a place of their choosing. Our specialist teams can visit you in your own home and work alongside your whānau and support network to help ensure you feel safe, comfortable, and well cared for.
Aged residential care
For those patients living in an aged residential care, we work in partnership with your care team. We offer clinical guidance and ongoing education to support staff in providing you with quality palliative care.
Rural communities
For people living in the rural communities of Wairoa and Central Hawke’s Bay we will work collaboratively with local health providers, including GP’s and district nurses. We provide specialist advice, support, and education to ensure high-quality care is received close to home.
InPatient Unit
Our Inpatient Unit (IPU) is a short‑stay service for people whose symptoms are difficult to manage at home or are in their last days of life.
In hospital
For patients admitted to hospital in Hawke’s Bay, we will work closely with the hospital palliative care team to ensure consistent care and coordinated discharge planning.
Outpatient
For specialist assessment and follow-up patients may be seen in their home, or they may attend an appointment at Cranford Hospice with a doctor, nurse practitioner, or specialist palliative care nurse, depending on their needs.
